I'm reading it right now. I'm from that demographic, as well as from the Ozarks, and I went to high school in the town he lived in.
I'm involved in a FB group conversation about it right now, a group created specifically for other Appalachians to discuss what Vance got right and got wrong (lots of both). Most of the people in the group are like me -- we "got out." Our sometimes-guilt-sometimes-glee about this adds a layer of complexity to our conversation.
I study the farming in the Balkans and former Yugoslavia, and I visit Croatia, Slovakia, and border regions of Bosnia regularly to interview people and live among them as they work on their farms and orchards. It surprises people when I tell them that Vance's writing is very like my ethnographic field notes at times. The largest difference, in my opinion, has to do with the despair.
